What to Bring to Your Appointment

To ensure your visit with us is as smooth and efficient as possible, please bring the following items with you to your appointment:

  • Bring your current insurance card(s). If you have more than one type of insurance, please bring both your primary and secondary cards.
  • If your insurance requires a referral, ensure it is processed before your visit.
  • A valid form of identification, such as a driver’s license, state ID, or passport.
  • A complete list of all medications you are currently taking, including prescription drugs, over-the-counter medications, and any supplements or vitamins.
  • If you wear glasses, bring them with you.
  • If you wear contact lenses, bring your contact lens prescription or a supply of your lenses and the name of the lens solution you use.
  • If you are transferring from another provider or have had previous eye care, please bring any relevant medical records or test results.
  • For co-pays or payments not covered by your insurance, bring a form of payment (credit card, debit card, or check).
  • If you are having a dilated eye exam, your eyes may be sensitive to light afterward. Bring a pair of sunglasses to wear after your appointment.
  • If another doctor referred you to our office, please bring any referral letters or notes.
  • Prepare a list of any questions or concerns you have about your vision or eye health so we can address them during your visit.
